About the position

The Behavioral Health Care Coordinator at UnitedHealthcare plays a crucial role in coordinating long-term care needs for patients within the community. This position is integral to an Integrated Care Model, focusing on advocating for patients and ensuring they receive the appropriate services to meet their health care needs. The role involves working closely with a dynamic team of Behavioral Health Specialists, RNs, and Social Workers to help members achieve their health care goals, while also providing flexibility through telecommuting options in designated regions.

Responsibilities • Assess, plan, and implement individualized care strategies for patients. , • Identify and initiate referrals for social service programs, including financial and psychosocial support. , • Manage the care plan throughout the continuum of care as a single point of contact. , • Communicate health-related information to all stakeholders to ensure quality coordinated care. , • Advocate for patients and families to ensure their needs and choices are represented. , • Visit Medicaid members in their homes and other settings, including community centers and hospitals.

Requirements • Must possess one of the following: RN in the state of Texas, Licensed Professional Counselor (LPC), Licensed Bachelor Social Worker (LBSW), or Licensed Masters Social Worker (LMSW). , • 2+ years of experience working within the behavioral health setting. , • Intermediate proficiency in MS Word, Excel, and Microsoft Outlook. , • Reliable transportation and ability to travel up to 75% of the time in the assigned region. , • Access to a designated quiet workspace at home with the ability to secure Protected Health Information (PHI). , • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection.
